The Alaska DMV offers limited licenses to eligible applicants who need to drive to work while their regular licenses are suspended or revoked. After a DUI conviction, you may be eligible to drive for the final 60 days of your license revocation period if your ability to earn a livelihood will be severely impaired without being able to drive. 

Steps to Obtaining a Restricted Driver’s License During Your Suspension:

  • Fill out the Limited License Form 404C and pay the $100 fee.
  • Provide proof of SR-22 insurance and proof from the Alcohol Safety Action Program (ASAP) that you have completed your treatment program.
  • Make an appointment to have your ignition interlock installed by calling toll-free 800-499-0994.
  • Have your interlock installed. Guardian will give you a certificate confirming the installation.
  • Bring the certificate to the DMV to receive your restricted license.

Steps to Reinstate Your Driver’s License After Suspension:

After serving your suspension or revocation period completely, you may reinstate your driving privileges and apply for a new Alaska driver’s license.

  • Visit your local Alaska DMV office and:
  • Complete an Application for Alaska Driver License, Permit or Identification Card (Form 478).
  • Pass all required tests (e.g., written, vision, road).
  • Submit proof of SR-22 insurance.
  • Present proof of birth and identity, such as your U.S. birth certificate (original or certified copy), current U.S. passport, or Alaska ID card. For a complete list of acceptable identification, please visit the Alaska DMV website.
  • Show completion of an alcohol treatment program, if required.
  • Make an appointment to have your ignition interlock installed by calling toll-free 800-499-0994.
  • Have your interlock installed. Guardian will give you a certificate confirming the installation.
  • Bring the certificate to the DMV to receive your restricted license.
  • Pay all required fees to obtain your reinstated license.
